Job Description:

Under the direction of the Vice President of the CBI Programs and direct supervision of the Director of Cayuga Family Resiliency, CPST Counselor will provide intensive home, school and community based counseling, crisis intervention/avoidance, case management, strength-based treatment planning, and psychoeducation to children and families.

HOURS PER WEEK:

Flexible hours, weekend availability required

FUNCTIONS:

  • Work in conjunction with an OLP clinician
  • Provide goal-directed and solution-focused interventions to youth and families to address challenges associated with behavioral health needs and to achieve identified goals or objectives as set forth in the child?s treatment plan
  • Assist the child and family/caregiver to achieve stability and functional improvement in daily living, personal recovery and or resilience, family and interpersonal relationships in school, and community integration
  • Actively participates as part of a team of service providers and others with focus on well-coordinated services for children and family members
  • Assists individuals and families by actively referring them to appropriate stabilizing services
  • Must be ready, willing and able to assist the families with any and all tasks needed to stabilize them with the intention of modeling and teaching basic life skills to support children and families to become self-sufficient
  • Performs the basic accountability tasks related to the job which include compiling and maintaining all necessary records, forms and reports
  • Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (eg., computer access, conversation, document) during and surviving the termination, expiration or cancellation of employment/association with CHC.
  • Provides on-call support as assigned
  • Assists administrative staff in collection of information for program effectiveness
  • Responds in a positive and respectful manner to diversity among the agency?s service population and staff
  • Provide crisis interventions defusing problem situations on a 24-hour a day on-call basis
  • Coordinates and facilitates parent group/adolescent group
  • Attend required training

  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. These essential job functions dictate the level of access, use, and disclosure of confidential information
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer
  • Experience and Skills:

    SKILLS/QUALIFICATIONS:

  • Licensable master?s degree in social work, counseling, or other relevant human service profession, with two years of experience in children and family services
  • Ability and willingness to work with families in an "in-home" context.
  • Knowledge of family systems theory and home based interventions
  • Ability to engage families who may not be successful in clinic settings
  • Ability to handle confidential material in a responsible manner
  • Ability to track units of service for billing
  • Maintain a caseload and follows required number of direct service contacts
  • Ability to have flexibility in hours worked both routinely, so as to meet children and families when they are available, and occasionally, in response to crises which may occur at any hour of any day.
  • Vehicle required: Must be safe and dependable.
  • PHYSICAL REQUIREMENTS:

  • Sitting for extended periods of time
  • Walking up and down stairs inside/outside
  • Must have ability to type and hand-write documentation and maintain proper files.
