Community Healthlink (CHL) is looking for a relief (0-20 hrs/wk) Crisis Support Worker/Family Partner in Northern Worcester County. Staff at Youth Mobile Crisis Intervention Services (YMCIS) work as a team to provide emergency response to youth and families experiencing a behavioral health crisis in community-based settings including homes, residential treatment centers, schools, and hospital emergency departments. The team provides an emergency intervention which includes the assessment of clinical and support services the family requires for stabilization in the community. Disposition could be referral for higher level of care, collaboration with current providers, referral for services and supports. As indicated YMCIS can continue to provide services for 7 days to aid with the stabilization process. The YMCIS Family Partner/Family Crisis Support Worker works along side crisis team clinicians, psychiatry, and other ESP staff as a member of a multi-disciplinary team devoted to linking the youth and their family to the appropriate level of care. + Short-term clinical crisis intervention service that is a mobile, on-site, face-to-face therapeutic response to a youth experiencing a behavioral health crisis. + Identifying, assessing, stabilizing and treating the youth and their family with the goal of reducing imminent risk and increasing coping skills and functioning. + Diverting youth from more intensive levels of care and maintaining youth in their home communities. + Assist the youth and family to engage and actively participate in the crisis intervention. + Knowledge of community resources and services they will often act as a bridge between families, agencies and community resources + Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors. + Grade 5: Family Partner I: Associates degree in a human services related field from an accredited school and one (1) year of experience working with youth/families; or High School diploma, HiSet or GED and a minimum of two (2) years experience working with youth/families + Grade 6: Family Partner II: Requires a Bachelor s degree; preferably in the field of psychology or human services from an accredited university or college and one (1) year of experience working with youth/families. + Experience as a caregiver of a youth with special needs preferably a youth with mental health needs + Experience in navigating and working with any of the child/family serving systems or teaching other parents/caregivers to navigate such systems OR + Crisis support worker-Requires a Bachelor s degree; preferably in the field of psychology or human services with 1 year of experience + Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work related purposes, including transporing clients in your own vehicle. + Must be able to pass a CORI background check Community Healthlink is an Affirmative Action/ Equal Opportunity Employer.
