Vacation, Sick and Holidays after 90 days Email Me Similar JobsEmail Me This Job Case Manager - ROP (Bronx) The Rights of Passage (ROP) Case Manager provides supportive services to 10-20 young people aged 16-21 living in a ROP site in the Bronx. Reporting to the Program Coordinator of the ROP site, the Case Manager is an energetic, creative, and diligent member of a team that includes resident advisors (who also have cooking responsibilities), and a cadre of specialty staff including employment/education specialists, housing specialists, youth development staff, and mental health staff. ROP is a transitional living program funded by the NYC Department of Youth and Community Development, where youth may reside for up to 24 months. The Case Manager will provide comprehensive case management services to the youth who are on their caseload including a creation of an individual service plan, coordination of services, individual counseling, advocacy, monitoring of the youth's efforts and challenges in completing goals, and celebrating goal achievement. The Case Manager will contribute to a program culture that is person-centered, holistic, focused on positive youth development, and committed to continuous improvement. The Case Manager is expected to participate in youth development and other therapeutic and life skill activities for the benefit of the youth living at the site, as well as staff meetings including case review meetings to assess youths' progress. Ultimately, Case Managers assist in a holistic way the achievement of goals of ROP: stabilize and address any immediate trauma experienced by the youth in our care, maintain housing stability, and assist the youth in advancing towards their individualized goals including a long-term housing plan, while ensuring the site is well-maintained. Additionally, this program will utilize evidence-based and other proven models; the Case Manager will be expected to participate in all training and implementation activities as needed. As these sites are new to CHNY, the Case Manager will need to be comfortable in a rapidly changing environment where processes and protocols are being developed and refined as the program is operating. Additionally, this position requires independent judgment and proactivity, as the Case Manager will be working within a newly opened, stand-alone site. Flexibility, a willingness to be available by email or phone in off-hours, and excellent communication and problem-solving abilities are critical to successfully fulfilling the requirements of this position. Duties and responsibilities + Case Management + Provides integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability and accomplishment of service plan goals on an ongoing basis. + Collaborates with youth to assess strengths and challenges and develop an individual service plan that reflects the youth's progress and stated goals. + Meets with youth regularly to determine progress with goals identified in the ISP. + Advocates on behalf of youth with outside service providers and other collateral services. + Creates and maintains confidential records, charts, and logs for youth documenting all relevant information from individual and group interventions as well as with collateral services. + Ensures that youths' files are compliant with all regulatory and other requirements. + Completes incident reports per agency policies. + De-escalates crises; attend to emergencies following agency protocol, as needed. + Team Participation + Works as a collaborative member of the team ensuring all youth have access to high-quality services and are living in a safe and compassionate community. + Participates in and facilitates regular case conferences. Ensures team members are fully apprised of important information regarding youth as needed. + Participates in staff team meetings and trainings, individual supervision meetings, and meetings with youth. + Assists with audit preparation and dry and live audits of the program. + Engages with youth during mealtimes, assisting with meal preparation and clean up, and other recreational activities and/or downtime at the site. + Professional Development and Embracing CHNY's Mission + Participates in all required trainings, either in-person or via CHNY's online Learning Management System (Relias). + Actively participates in the training and implementation of evidence-based and other proven models. + Embraces a data-driven culture that is outcomes-oriented and creatively responsive to programmatic challenges. + Promotes a healthy, inclusive and equitable organizational culture and a thriving, engaged staff. + Promotes child protection within the site and is diligent about the safety of clients and fellow staff members. + Always uphold CHNY's mission, vision, and principles, standards of conduct, policies and procedures. + Perform other duties as assigned and needed. Qualifications + Bachelor's degree required, preferably in a relevant field. + One (1) year experience in a residential setting working with adolescents preferred. + Demonstrated knowledge of and experience working directly with young people through approaches that emphasize resiliency, strength-based youth development, and trauma-informed care required. + Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required. + Excellent verbal, written and problem-solving abilities and an upbeat demeanor required. + A strong focus on child protection and ensuring the safety of residents required. + Experience working with diverse groups required. + Overall flexibility in work schedule required, including weekends, evenings and holidays if needed. + Experience utilizing evidence-based and other social service models preferred. + Dedicated to the mission of Covenant House.
