Provide therapy and care coordination services to teens with substance use disorders and mental health issues in a residential treatment program. You have the opportunity to change the lives of youth and their families, while working along side a dedicated multi-disiplinary team.

This co-ed,14-bed facility is currently located in Chisago City MN, with plans to relocate to Maplewood MN this summer, June 2020.


Job Summary :

Conducts multi-axial diagnostic assessment, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary team and all other contacts. Functions as an interdisciplinary team member. Accepts supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice.

Provide individual, group, multifamily groups, and family therapy with adolescents in an IOP and residential treatment program.

Job Expectations:

Demonstrates clinical competence in the treatment planning process.

  • Facilitates treatment planning and oversee management of each case.
  • Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team.
  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjust treatment plan as necessary.
  • Demonstrates clinical competence to assess individual patient needs.

  • Completes an interdisciplinary assessment tool with each patient.
  • Updates interdisciplinary assessments tool each week.
  • Demonstrates clinical competence in disposition planning.
  • Interfaces with government agencies such as court, ombudsmen, county services
  • Completes referral process for next level of care
  • Communicates to other providers in the community
  • Sets up intake appointments
  • Completes needed paperwork to facilitate this process i.e. applications, letters of recommendation, etc
  • Demonstrates clinical competency to partner with multi-disciplinary team in the treatment planning process.
  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts treatment plan as necessary.
  • Communicates frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
  • Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
  • Demonstrates competence in developing a clinically relevant transition/discharge plan.
  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan
  • Develops discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups.
  • Provides individual, couples and family therapy, multifamily groups
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities:
  • Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
  • Utilizing evidence based policies in clinical practice
  • Qualifications

    Required Qualifications:

  • LICSW, LP, LPCC or LMFT licensure
  • Experience providing counseling to adolescent population
  • 30 hours of Adolescent development course work or training
  • 150 hours of supervised adolescent experience
  • Basic Life Support Certification
  • Preferred Qualifications:

  • LADC licensure
